Article 1
An employee working in a mixer at a concrete batch plant was struck and crushed by rotating tines on the equipment when it was accidentally activated by a co-worker. None of the employees at the temporary facility had received training in lockout.
Article 2
A carpenter, engaged in the construction of temporary partitions around an elevator shaft, was decapitated when an elevator car he assumed to have been locked out descended and caught his head between the car frame and the barrier he was building. He had a "verbal agreement" with the elevator constructor on the previous day that cars would be deactivated until all barriers were constructed. Power disconnect apparatus for each elevator car in the building's penthouse was equipped with a feature for the attachment of a padlock.
Article 3
An electrician making changes to a temporary service in a building under construction believed power to be off because he opened circuits at the breaker panel and attached a tag on its door. His tag clearly warned that lines should not be energized until he removed it. An employee of another craft, unable to operate his power tool, proceeded to the breaker panel and began closing circuits. The electrician, whose body was in contact with metal piping, was fatally electrocuted. The doors of the breaker panel and the closet where it was located were both equipped with locks.

A fitter working in the confines of a high pressure line at a nuclear power plant under construction was asphyxiated when a valve on the other end of the pipe was opened and an inerting gas was introduced.
