Dear Member:
Re: GROUP ACCIDENT INSURANCE PLAN
We are pleased to report to you that MCAA Group Insurance Plans during the past year increased their enrollment over 40%. This shows that they are being widely accepted as a valuable aid.
Another policy anniversary date is approaching and we ask that you review your current coverage with the possibility of increasing your participation.
This 24-hour "All Risk" Accident insurance (in addition to the $1,000 free coverage provided by MCAA to one individual in each member-firm) is open to MCAA members, officers, partners and salaried employees. Dependents of male members, officers, partners or salaried employees may be included, if desired. Wives are subject to a $50,000 maximum limit, children to a $10,000 maximum limit; dependent coverage limits cannot exceed amount carried by the husband.
As an individual, $10,000 Accidental Death & Dismemberment Insurance would cost you $17.50 per year but you pay just $8.50 through MCAA.
Now is the time to send in new names to be added effective December 1, 1968, and to increase the limit carried by those individuals presently insured.
Limit | Annual Premium Per Person Insured
------- | --------
$ 10,000 | $ 8.50
25,000 | 21.25
50,000 | 42.50
75,000 | 63.75
100,000 | 85.00
The principal amount is payable in event of accidental death; dismemberment amounts according to percentage schedule to be provided.
For proper service please address all checks and request for coverage to Walter T. Derk, Fred S. James & Co., 1 North LaSalle Street, Chicago, Illinois 60602.
